The first thing that you want to do is find an image of the bottle. Preferbably a good front shot. Save it and now open your given 3d application.

To maximise the realism of your 3D models we recommend using high quality photo references from the #1 texture website www.environment-textures.com
Create a box in the Front viewport, like so, and apply the dimensions as follows:
Length: 55.0Width: 55.0Height: 99.0
Then rotate the box on the Y axis 90 degrees.

Next, apply an edit poly modifier and select the Polygon sub-object.
Select the top polygon and inset it to the amount of 3.0.

Now you want to extrude the top polygon several times like the image below

Next you need to select all of the edges and apply a Chamfer in the amount of 1.0
Now for the top cap of the bottle. Create a cylinder with the dimensions of :
Radius: 11.5Height: 18.0
Next, apply a Edit Poly modifier to it, and then select the top and bottom row of edges and chamfer them to the amount of .5.
Now apply a MeshSmooth modifier and leave the Iterations at 1.
